Reports to:  Registered Nurse/Director of Care

Job Description:

  • Practices as a member of the multi-disciplinary team, under the standards of the Ministry of Health, the College of Nurses of Ontario and the policies of the Home;
  • Maintains knowledge of and promotes in all aspects of resident care the Resident Bill of Rights;
  • Supervises, directs and assists the Personal Support Workers in the provision of personal care;
  • Assists in implementing and enforcing facility and departmental policies;
  • Under the supervision of the Registered Nurse, maintains the safekeeping of all drugs and prescribed treatments on the unit; administers drugs and treatments according to procedure;
  • Supports and actively participates in orientation, in-service programs and other staff development;
  • Assists the Director of Care in evaluating the performance of Personal Support Workers; informs the Registered Nurse of staff performance issues;
  • Under the supervision of the Registered Nurse, participates in quality improvement and risk management, including leadership of initiatives as assigned; conducts ongoing audits for effectiveness;
  • Supports and actively participates in orientation, in-service programs and other staff development;
  • Assists the Director of Care in evaluating the performance of Personal Support Workers; informs the Registered Nurse of staff performance issues;
  • Under the supervision of the Registered Nurse, participates in quality improvement and risk management, including leadership of initiatives as assigned; conducts ongoing audits for effectiveness;
  • Under the direction of the Registered Nurse,  is responsible for providing nursing care at the skill level compatible with the RPN scope of practice, personal assistance and behavioural supervision to residents experiencing behaviours associated with complex and challenging mental health and/or physical impairment;
  • Participates in the preparation and maintenance/updating of resident care plans;
  • An effective team-member and leader, facilitates coaching/mentoring of fellow staff and contributes to assessment of dementia, cognitive impairment and delirium, mental health, addictions, physical assessment and techniques of behavioural approach and evaluation, implementing best evidence-based clinical practice in provision of patient care within their scope of practice in collaboration with multidisciplinary team, including allied and inter-professional care teams
  • Other related duties as assigned.

Skills/Education Required:

  • Must possess and maintain a current Certificate of Competence as an RPN issued by the College of Nurses of Ontario;
  • Must have advanced training, knowledge and leadership experience in wound management;
  • Demonstrated customer-service skills;
  • Certificates or demonstrated training in P.I.E.C.E.S.™, Gentle Persuasive Approach, U-First! and Montessori preferred;
  • Excellent critical thinking skills and a thorough understanding of pharmacological agents/interactions and non-pharmacological and therapeutic behaviour approaches;
  • Must be able to deal tactfully and professionally with sensitive situations, residents and their families and at all times exercise discretion, judgment, diplomacy, and confidentiality, respecting resident rights and privacy of information while in an advocacy role;
  • Must have excellent organizational and administrative (documentation and computer) skills and be able to work effectively and co-operatively in a multi-disciplinary team environment, or independently.
  • Employees must be willing to work weekdays and weekends, all shifts including days, afternoons, and nights, and in any of the four houses as assigned.
  • Hours of work may be days, evenings, nights, (full & short shifts) in Victoria, Elford, Vaga and McMillan House, subject to change from time to time to meet operational requirements.  Accordingly, the employee must be willing and able to work weekdays and weekends, all shifts including days, afternoons, and nights, and in any of the four houses as assigned. Human Resources will identify location and shift during call out process.
